From 2018 to late 2024, the Friendship Bench expanded its footprint across Zanzibar, addressing a critical gap in mental health services. Through a dedicated partnership with Health Improvement Project Zanzibar (HIPZ) and the Ministry of Health, we worked to bring evidence-based mental health programme to support those struggling.

Culturally Rooted: Dawati Rafiki

To ensure the program resonated with the local community, the initiative was introduced as Dawati Rafiki (The Friendship Bench in Swahili). This localization involved adapting the SSQ-14 screening tool and counseling approach to align with the cultural nuances in Zanzibar, making mental health support approachable and stigma-free. Those who scored 8 or above were offered six sessions of talking therapy

Six Years of Impact

The project served as a vital bridge for thousands of individuals across Unguja and Pemba who previously had no access to to support at the primary healthcare level.

  • Active implementation from 2018 to December 2024.

  • Successfully trained 300 Community Health Volunteers (CHVs) in the Friendship Bench model, equipping them with Problem Solving Therapy (PST) skills.

  • The program was integrated into 32+ primary healthcare clinics, providing a lifeline to those experiencing "thinking too much" or emotional distress.

In December 2024, the project officially drew to a close. While the formal project cycle has ended, its impact remains embedded in the community. The 300 volunteers trained during this period remain a lasting asset to Zanzibar, now equipped with the tools and experience to identify, respond to, and support mental health needs within their own neighborhoods.

The journey in Zanzibar shows the power of community-led interventions and the enduring strength of the "Dawati Rafiki" spirit.
